高建忠运用温胆汤调畅三焦

温胆汤的功用虽以中焦脾胃为基础,但不拘于中焦,可调畅三焦治疗全身性病症,常与调畅一身气血的血府逐瘀汤,调和阴阳表里三焦的柴桂龙牡汤,治六郁的越鞠丸等方合用。临床中"郁"往往不是单一的,如痰郁合血瘀、痰郁合气滞、痰郁合食积等。高建忠老师治疗上重视通郁,调畅三焦,常以通郁祛邪为先,在用温胆汤理气化痰以通痰郁的基础上,常合用青皮、香附、柴胡、郁金、枳实、合欢花、甘松等解郁之品。同时,立足中焦脾胃,治病求本,注重对后天之本的顾护,不仅是补脾,更多是运脾。温胆汤为立足脾胃,侧重祛邪通郁的方子,不仅可通过理气化痰解肝胆之郁,亦可通过降胃气、补脾气促进中焦气机恢复,从而达到调畅全身气机的作用。
