血清趋化因子CXC配体16 热休克蛋白70 高迁移率族蛋白B1及可溶性CD40配体与高血压合并脑梗死病灶 神经功能的关系

摘    要:目的探讨血清趋化因子CXC配体16(CXCL16)、热休克蛋白70(HSP70)、高迁移率族蛋白B1(HMGB1)、可溶性CD40配体(sCD40L)在高血压合并脑梗死患者急性期病情变化中的意义。方法选取郑州市第三人民医院确诊的110例高血压合并脑梗死患者为脑梗组,单纯高血压患者110例为对照组,检测2组血清CXCL16、HSP70、HMGB1及sCD40L水平及一般生化资料;按照脑梗死病灶大小、NIHSS评分分为小梗死组、中梗死组、大梗死组及轻度组、中度组及重度组,并进行分析。结果脑梗组血清CXCL16、HSP70、HMGB1及sCD40L高于对照组(P<0.05);小梗死组、中梗死组、大梗死组血清CXCL16、HSP70、HMGB1及sCD40L呈递增趋势,组间对比差异有统计学意义(P<0.05);轻度组、中度组及重度组血清CXCL16、HSP70、HMGB1及sCD40L呈递增趋势,组间对比差异有统计学意义(P<0.05)。结论高血压合并脑梗死患者CXCL16、HSP70、HMGB1及sCD40L较单纯高血压患者显著升高,并与脑梗死病灶大小、神经功能受损程度有关。

Abstract:Objective To explore the significance of serum chemokine CXC ligand 16(CXCL16),heat shock protein 70(HSP70),high mobility group protein B1(HMGB1),soluble CD40 ligand(sCD40L)in patients with acute hypertensive cerebral infarction.Methods The serum levels of CXCL16,HSP70,HMGB1 and sCD40L and general biochemical data were measured in 110 patients with cerebral infarction and hypertension(cerebral infarction group)and 110 patients with hypertension as control group.Subgroup analysis was carried out according to the size of cerebral infarction focus and NIHSS score.Results The serum levels of CXCL16,HSP70,HMGB1 and sCD40L in cerebral infarction group were higher than those in control group(P<0.05);the serum levels of CXCL16,HSP70,HMGB1 and sCD40L in cerebral infarction group with large infarction,medium infarction and small infarction increased gradually,and there were significant differences between the two groups(P<0.05);the serum levels of CXCL16,HSP70,HMGB1 and sCD40L in mild,moderate and severe cerebral infarction patients were higher than those in control group(P<0.05).The difference between the two groups was statistically significant(P<0.05).Conclusion The levels of CXCL16,HSP70,HMGB1 and sCD40L in patients with cerebral infarction and hypertension were significantly higher than those in patients with hypertension alone,and were related to the size of cerebral infarction lesions and the degree of neurological impairment.
