天麻素联合尼莫地平治疗偏头痛急性发作期临床研究

目的探讨天麻素注射液联合尼莫地平片治疗偏头痛急性发作期的疗效及安全性。方法选取开封市中心医院神经内科收治的240例急性期偏头痛患者,随机分为对照组(120例)和实验组(120例)。对照组常规口服尼莫地平片,实验组在口服尼莫地平片的基础上加用天麻素注射液,对比分析2组患者在2周后的临床疗效、颅内血流速度及不良反应情况。结果经2周治疗,2组患者头痛发作频率显著降低、头痛持续时间缩短、头痛程度亦减轻,实验组总有效率91.67%,明显高于对照组的82.50%,差异有统计学意义(P=0.011)。TCD检测发现,实验组患者治疗后大脑前动脉和大脑中动脉血流速度明显低于治疗前和对照组,差异有统计学意义(P<0.05);实验组不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论天麻素注射液联合尼莫地平片可明显提高偏头痛急性发作期的临床疗效,有效降低大脑前动脉和大脑中动脉的平均血流速度,提高患者生活质量,且不良反应少。

Clinical study of gastrodine for injection combined with nimodipine in the treatment of acute migraine

Objective To investigate the efficacy and safety of gastrodine for injection combined with nimodipine tablets in the treatment of acute migraine.Methods 240 patients with acute migraine who were admitted and treated by neurology were randomly divided into control group(120 cases)and observation group(120 cases).The patients in the control group orally took nimodipine tablets,while the observation group orally took nimodipine tablets and Tianma Capsule for 2 weeks.To compare the headache frequency,headache duration,headache degree,efficacy;the average flow velocity of middle cerebral artery,anterior cerebral artery,posterior cerebral artery,vertebral artery and basilar artery;adverse reactions and quality of life in two groups before and after treatment.Results After 2 weeks of treatment,two groups of patients with headache frequency decreased significantly,headache duration,also reduce the degree of headache,the experimental group the total effective rate was 91.67%,significantly higher than the control group total effective rate was 82.50%,the difference is statistically significant(P=0.011).TCD examination found that the experimental group after treatment in patients with MCA and ACA blood flow velocity obviously lower than that before treatment and the control group statistically significant difference(P<0.05).The experimental group significantly lower than that of control group in incidence of adverse reactions incidence of adverse reactions,significant difference statistically significant(P<0.05).Conclusion Gastrodia capsule with nim horizon combination therapy can obviously improve the clinical curative effect of migraine patients use nim is obviously better than plain film,can effectively reduce the average blood flow velocity of AC and MC,improve the quality of life of patients,and the low incidence of adverse reactions,worthy of popularization and application in clinic.
