丁苯酞结合早期介入下肢重复性训练在脑梗死康复中的应用

摘    要:目的探讨丁苯酞结合早期介入下肢重复性训练应用于脑梗死康复中的效果。方法选取脑梗死患者74例,按照随机数表法分为研究组和对照组各37例,均给予常规治疗,对照组给予早期介入下肢重复性训练,研究组在对照组的基础上给予丁苯酞治疗。比较2组的治疗疗效,治疗前后的丙二醛(MDA)、超氧化物歧化酶(SOD)、脑源性神经生长因子(BDNF)、神经元特异性烯醇化酶(NSE)水平以及美国国立卫生研究院脑卒中量表(NIHSS)、Fugl-Meyer运动功能评分法(FMA)、Barthel指数(BI)评分。结果研究组治疗总有效率为89.19%,明显高于对照组的64.86%(P<0.05)。治疗前2组MDA、SOD、BDNF、NSE水平比较差异无统计学意义(P>0.05);治疗后2组MDA、NSE均降低(P<0.05),SOD、BDNF升高(P<0.05),且研究组MDA、NSE较对照组降低更显著(P<0.05),SOD、BDNF较对照组升高更显著(P<0.05)。治疗前2组NIHSS、FMA、BI评分比较差异无统计学意义(P>0.05)。治疗后2组NIHSS评分降低(P<0.05),且研究组较对照组降低更显著(P<0.05);2组FMA、BI评分升高(P<0.05),且研究组较对照组升高更显著(P<0.05)。结论丁苯酞结合早期介入下肢重复性训练应用于脑梗死康复中治疗效果显著,可有效抑制氧化应激反应,改善神经功能缺损、日常生活能力,提高运动能力。

Abstract:Objective To investigate the effect of butylphthalide combined with early interventional lower limb repetitive training in the rehabilitation of cerebral infarction.Methods A total of 74 patients with cerebral infarction were selected and divided into a study group and a control group with 37 cases each according to the random number table method.Both were given conventional treatment.The control group was given early interventional lower limb repetitive training.The study group was given Ding on the basis of the control group.Phthalphthalein treatment.Compare the therapeutic effects of the two groups,the levels of malondialdehyde(MDA),superoxide dismutase(SOD),brain-derived nerve growth factor(BDNF),neuron-specific enolase(NSE),and national Institute of Health Stroke Scale(NIHSS),Fugl-Meyer Motor Function Score(FMA),Barthel Index(BI)score.Results The total effective rate of treatment in the study group was 89.19%,which was significantly higher than 64.86%in the control group(P<0.05).There was no significant difference in the levels of MDA,SOD,BDNF,and NSE in the two groups before treatment(P>0.05);MDA and NSE decreased in the two groups after treatment(P<0.05),SOD and BDNF increased(P<0.05),and the study group MDA and NSE decreased more significantly than the control group(P<0.05),and SOD and BDNF increased more significantly than the control group(P<0.05).There was no significant difference in NIHSS,FMA,and BI scores between the two groups before treatment(P>0.05).After treatment,the NIHSS scores of the two groups decreased(P<0.05),and the study group was significantly lower than the control group(P<0.05);the FMA and BI scores of the two groups increased(P<0.05),and the study group was higher than the control group More significant(P<0.05).Conclusion Butylphthalein combined with early interventional lower limb repetitive training has significant therapeutic effects in the rehabilitation of cerebral infarction.It can effectively inhibit oxidative stress response,improve neurological deficits,daily living ability,and exercise capacity.
