鼻咽癌放疗后多发肺转移治疗效果分析

目的回顾性分析单纯化疗、全肺放疗联合化疗治疗鼻咽癌放疗后多发肺转移患者的治疗效果,探讨较为合理的治疗方案。方法分析46例鼻咽癌放疗后多发肺转移患者的治疗方式及不同治疗方式对治疗效果的影响,其中单纯化疗27例,全肺放疗联合化疗19例,化疗方案主要是:顺铂加5-氟尿嘧啶、紫杉醇加顺铂;全肺放疗单次剂量120～150cGy,总剂量(DT)1 800～2 100cGy,均是未校正剂量。结果单纯化疗组与全肺放疗联合化疗组的有效率分别是44.44%和73.68%(P<0.05);单纯化疗组和全肺放疗联合化疗组6、12、18个月生存率分别是81.48%、29.63%、7.41%和89.47%、63.16%、42.11%,其中12个月和18个月生存率两组比较差异有显著性(P<0.05)。结论全肺放疗联合化疗治疗鼻咽癌多发肺转移的治疗效果优于单纯化疗。

The outcomes of treatment for multiple lung metastasis of nasopharyngeal carcinoma after radiotherapy

Objective To retrospectively analyze the efficacy of chemotherapy alone,whole lung radiotherapy plus chemotherapy in the treatment of multiple lung metastasis after radiotherapy for patients with nasopharyngeal carcinoma,and to explore more reasonable treatment options.Methods 46 patients with multiple lung metastasis by nasopharyngeal carcinoma after radiotherapy were treated with different regimen in our hospital,the response rates of different regimen were assayed.Of which 27 cases received chemotherapy alone,19 were given whole lung radiotherapy plus chemotherapy,the main chemotherapy regimen was: cisplatin plus 5-fluorouracil,paclitaxel plus cisplatin;single dosage of whole lung radiotherapy was 120~150cGy,total dosage(DT) 1800~2100cGy,all were uncorrected dosages.Results The radiotherapy plus chemotherapy for the lung metastasis had much better response rate than chemotherapy(73.68% vs.44.44%,P<0.05).The 6-month,12-month,and 18-month survival rates of chemotherapy alone and radiotherapy plus chemotherapy were 81.48%、29.63%、7.41% and 89.47%、63.16%、42.11%,respectively.Compared the 12-month and 18-month survival rates of chemotherapy alone to radiotherapy plus chemotherapy,there were statistically significant(P<0.05).Conclusion Radiotherapy plus chemotherapy in the treatment of multiple lung metastasis of nasopharyngeal carcinoma is better than chemotherapy.
