基质金属蛋白酶-2及9在胰腺癌组织中表达及意义

目的探讨基质金属蛋白酶（matrix metalloproteinase,MMP）-2及MMP-9在胰腺癌组织中表达情况,及其与胰腺癌临床病理参数及预后的关系。方法应用免疫组织化学方法检测行胰十二指肠根治性切除手术治疗胰腺癌患者79例癌组织及癌旁组织标本,及因胰腺外伤行胰腺部分切除10例患者正常胰腺组织标本中MMP-2及MMP-9表达。结果 MMP-2及MMP-9在正常胰腺组织中无表达或低表达,在胰腺癌组织中表达强度较癌旁组织增高（P〈0.05）;表达强度与肿瘤病理分级、TNM分期及预后均有相关性（P〈0.05）,但经Cox风险回归模型对病理分级、TNM分期进行调整后显示,MMP-2及MMP-9单独或联合表达均不能作为评估胰腺癌预后的独立预测指标。结论胰腺癌组织中MMP-2及MMP-9阳性表达与胰腺癌浸润、转移有关,但不能作为胰腺癌患者预后的独立预测指标。

Expressions of matrix metalloproteinase-2 and-9 in pancreatic carcinoma and its significance

Objective To investigate the expressions of matrix metalloproteinase（MMP）-2 and MMP-9 protein in the tissue specimens of pancreatic carcinoma（PC）,and the relationship with the clinical pathological features and their prognostic significance in PC.Methods The expressions of MMP-2 and MMP-9 protein were evaluated with immunohistochemistry in the carcinoma tissues and adjacent tissues in 79 patients receiving radical pancreaticoduodenectomy and in the normal pancreatic tissues in 10 patients receiving partial pancreatic resection due to trauma.Results The expressions of MMP-2 and MMP-9 were not found or lowly expressed in normal pancreatic tissues.The expressions of MMP-2 and MMP-9 were higher in pancreatic carcinoma tissues than those in adjacent tissues（P0.05）.The expressions of MMP-2 and MMP-9 were correlated with the pathological grade,TNM stage and PC prognosis（P0.05）.But after adjusting pathologic grade and TNM status by Cox proportional hazards regression models,neither of their alone nor combined expression was the independent prognostic marker for PC（P0.05）.Conclusion The expressions of MMP-2 and MMP-9 are correlated with invasion and metastasis.But neither of their alone nor combined expression can serve as an independent prognostic marker for PC.
