鲍曼不动杆菌的感染分布及耐药性分析

摘    要:目的 了解鲍曼不动杆菌感染在某院的临床分布及其耐药特点。方法 收集 2 0 0 0年 7月~ 2 0 0 3年 7月从临床感染标本中分离的 2 5 4株鲍曼不动杆菌 ,了解其临床分布并做药敏试验 ,比较其耐药率。结果  2 5 4株鲍曼不动杆菌临床分布以ICU最多 (1 1 8株 ,4 6 .4 6 % ) ,其次为呼吸病区 (4 0株 ,1 5 .75 % )。对三代头孢及环丙沙星的耐药率均 >5 0 .0 0 % ;对亚胺培南 ,美罗培南 ,及氨苄西林 /舒巴坦的耐药率较低 ,分别为 1 6 .93% ,2 2 .0 5 % ,2 2 .2 0 % ;发现 1 85 (72 .83% )株多重耐药菌株。各病区的感染菌株对部分抗菌药物的耐药性差异有显著性 (P <0 .0 5 )。结论 碳青霉烯类及氨苄西林 /舒巴坦对鲍曼不动杆菌仍有较强的活性 ,但不能忽视对这两类药物耐药的多重耐药菌株的出现。要减少多重耐药菌株的产生 ,应注意合理使用抗菌药物

Abstract:Objective To study the distribution and antimicrobial resistance of Acinetobacter baumannii infection in a hospital. Methods Two hundred and fifty four strains of Acinetobacter baumannii isolated from patients with infection from July 2000 to July 2003 were collected and antimicrobial susceptibility tests were performed. Results One hundred and eighteen strains ( 46.46% ) of Acinetobacter baumannii were from intensive care unit, forty strains ( 15.75% ) were from respiratory unit. The resistant rates of the strains to the third generation cephalosporin and ciprofloxacin were both more than 50.00%; the resistant rate to imipenem was the lowest ( 16.93% ), followed by meropenem (22.05%) and ampicillin/sulbactam ( 22.20% ); 185 strains ( 72.83% ) were multiply resistant to antimicrobial agents. The antimicrobial resistance of strains isolated from different wards had significant difference (P< 0.05 ). Conclusion Carbapenem and ampicillin/sulbactam still had a strong antimicrobial activity against Acinetobacter baumannii. Antimicrobial agents should be used rationally to decrease multiply resistance.
