富含缓释淀粉的肠内营养乳剂对2型糖尿病血糖及胰岛素的影响

摘    要:目的 观察富含缓释淀粉的肠内营养乳剂对 2型糖尿病患者血糖、血浆胰岛素曲线下面积及空腹血糖的影响。方法  30例 2型糖尿病患者 ,采用前后自身对照的方法比较第 1天 (服用对照标准早餐 )和第 2、3天 (以“瑞代”作为早餐 )空腹和餐后血糖以及胰岛素水平的变化。结果 第 3天的空腹和餐后 1h血糖水平明显低于第 1天 (P <0 .0 1) ,血糖曲线下面积 (AUCG)也明显下降 (P =0 .0 0 5 ) ,但餐前及餐后胰岛素曲线下面积(AUCI)的差异无显著性 (P =0 .86 1) ;另外研究发现胰岛素抵抗稳态模型分析 (HOMA IR)呈下降趋势 (P =0 .0 2 1)。结论 富含缓释淀粉的肠内营养乳剂与对照标准早餐相比 ,能有助于改善胰岛素的抵抗 ,降低空腹及餐后血糖 ,且无需改变糖尿病患者的治疗方案 ,是一种适合于 2型糖尿病患者服用的安全的肠内营养乳剂。

Abstract:Objective To evaluate the impact of slow-release starch containing enteral nutrition on blood glucose levels, the areas under the curve of blood glucose and insulin values in type 2 diabetic patients.Methods In this self-control study, 30 type 2 diabetics had different kinds of breakfast for three days. Compared with the first day (standard meal), fasting/postprandial blood glucose and insulin levels were assessed on the next two days (Fresubin diabetic diet).Results On the third day, fasting and 1 hour postprandial blood glucose were significant lower than those of control on the first day(P <0.01)as well as diminished in areas under the curve(P= 0.005).But the difference of the area under the curve of insulin between pre and postprandial was not significant(P= 0.861). Besides, HOMA-IR index showed decrease tendency(P= 0.021).Conclusions Compared with the standard meal, slow-release starch containing enteral nutrition can be helpful for type 2 diabetics in improving the insulin resistance, decrease of fasting and postprandial blood glucose without changes in antidiabetic therapy.
