Wallenberg综合征12例临床分析

目的：探讨Wallenberg综合征的病因、临床特点及预后。方法：对12例Wallenberg综合征患者的临床资料进行分析。结果：12例患者均为梗死所致，以眩晕、恶心、呕吐起病者多，延髓麻痹和感觉障碍为主要表现，头部MRI阳性率100％，经治疗眩晕、恶心、呕吐、声音嘶哑及吞咽困难恢复较快，感觉障碍恢复最慢。4例基本痊愈，3例显著进步，1例无变化，2例病情恶化，2例病后3d～7d因颅压高、上消化道出血、心跳骤停而死亡。结论：动脉粥样硬化致椎基底动脉血栓形成是Wallenberg综合征的主要病因但不是唯一原因。本病并发症多，致残率高，预后差，应及早严格识别。

Clinical Analysis of 12 Cases with Wallenberg Syndrome

Objective: To explore the causes,clinical characteristics and prognosis of Wallenberg syndrome.Methods:The clinical data of 12 patients with Wallenberg syndrome were analyzed.Results:All 12 patients were all caused by infarctus.Most of them had dizziness,nausea and vomiting at the early stage of onset and their principal symptoms were bulbar paralysis and sensory disturbance.The positive rate of head MRI were 100%.After treatment the symptoms of dizziness,nausea,vomiting,hoarseness and dysphagia were relieved quicklier,while the symptom of sensory disturbance was relieved slowliest.Seven cases improved and healed,1 case was unchanged,2 cases were exacerbation and 2 cases died of intracranial hypertension,upper gastrointestinal bleeding and cardiac arrest in three to seven days after illness.Conclusions: Vertebral basilar artery thrombosis caused by atherosclerosis is the main cause of Wallenberg syndrome but not the sole reason.This syndrome has many complications.For example,its disablement rate is high and the prognosis is bad,so we should recognize it early and strictly.
