347例老年患者口服抗心律失常药用药分析

目的：调查我院临床治疗老年患者心律失常的用药习惯及合理性，为抗心律失常药临床合理应用提供数据。方法：利用《合理用药临床药学工作站》系统，在我院2009年1～12月住院明确诊断心律失常的老年患者中，筛选出使用口服抗心律失常药的病例，记录患者例数、年龄、心律失常临床表现、合并的主要疾病、药物使用方法、剂量等，依据相关资料进行用药合理性判断。结果：抗心律失常药比索洛尔、美托洛尔、美西律及普罗帕酮在治疗房颤、房性或室性早搏时多采用了较小剂量；药物的用法规范，尤其是胺碘酮，在治疗房颤时符合中华医学会心血管病学分会推荐的慢负荷方法；在用药的选择上虽然符合选用原则，但部分用药还缺乏大样本的临床循证医学依据。结论：我院老年患者抗心律失常药的运用基本合理。

Analysis of the Use of Oral Anti-arrhythmic Drugs for 347 Elderly Patients

Objective ： To investigate the usage and the rationality of antiarrhythmic drug in the clinical treatment of elderly patients in our hospital to supply data for the clinical management of the rational dru use. Methods： With the ＂workstation of rational use on clinical pharmacy＂ system, the relevant information of number, age, clinical arrhythmia, merger diseases, usage, dosage and cases were selected from hospitalized elderly patients with arrhythmia and oral anti - arrhythmic drugs in our hospital from January to December of 2009 and the rationality of medication, judged. Results： The doses of bisoprolol, metoprolol, mexiletine and propafenone in the treatment of atrial fibrillation, atrial or ventricular premature beats were low but the usage was normal. In particular, the usage of amiodarone hydrochloride in the treatment of atrial fibrillation met the slow-load method which was recommended by the Chinese Society of Cardiology. Although the selection in medication fulfilled the principle of choice, parts of drug uses still lacked clinical medicine-based evidence with large - samples. Conclusion ： The use of anti-arrhythmic drugs for the elderly patients in our hospital was rational.
