益生菌治疗溃疡性结肠炎疗效及对炎性细胞因子表达的影响

目的：探讨益生菌治疗溃疡性结肠炎（UC）的临床疗效及对UC患者外周静脉血炎性细胞因子IL-8、TNF-α的调节作用。方法：70例活动期UC患者随机分为两组，各35例，对照组给予口服美沙拉嗪缓释颗粒0．5g，pobid；治疗组在此基础上加用益生菌0．42g，餐前口服，tid。疗程均为8周。结果：治疗组显效率、总有效率分别为48．6％、100％，高于对照组（22．9％、91．4％），P〈0．05。治疗组镜下完全缓解率31．4％也高于对照组的14．3％（P〈0．05）。治疗后，两组外周静脉血IL-8、TNF-α水平均明显下降（P〈0．05）。治疗组下降水平更明显（P〈0．05）。结论：益生菌对UC患者外周静脉血IL-8、TNF-α的异常表达有一定的调节作用，治疗UC疗效确切、安全可靠，有一定的临床推广价值。

Effect of Probiotics on the Expression of Inflammatory Cytokines in the Treatment of Ulcerative Colitis

Objective： To evaluate the clinical efficacy of probiotic treatment of ulcerative colitis （UC） and its regulatory effect on IL-8, TNF-α in peripheral blood of UC patients. Method ：70 patients with active UC were randomly divided into treatment group （35 patients） and controlled group （35 patients）. The patients in the treatment group received a combined treatment of mesalazine sustained-release granules, bifidobacterium longnm, lactobaeillus acidophilus, streptococcus faecalis and probiotics mixture, and patients in the controlled group were given oral mesalazine sustained-release granules. Result：The markedly effective rate and the total effective rate in the treatment group were 48.6% and 100%, while those in the controlled group were 22.9%, 91.4%, respectively. The efficacy of the treatment group was significantly superior to that of the controlled group （ P 〈 0.05 ）. The endoscopic complete response rate in the treatment group was 31.4%, while the control group was 14.3% （P 〈0.05）. After the treatment, IL-8 and TNF-α levels of the treatment group were significantly lower than those of the controlled group（ all P 〈0.05 ）. Conclusion： Probiotics had certain regulatory effects on the abnormal expression of IL-8 and TNF-α in peripheral blood of UC patients, being effective, safe and reliable and having its clinical value.
