可溶性血管细胞粘附分子1(sVCAM-1)在紫癜性肾炎发病中的作用探讨

摘    要:目的:探讨血管细胞粘附分子-1(VCAM-1)在过敏性紫癜性肾炎(HSPN)发病中的作用,并观察其与白介素-6(IL-6)、免疫球蛋白E(IgE)在HSPN发病中的关系。方法:采用酶联免疫吸附法(ELISA)检测27例过敏性紫癜、35例紫癜性肾炎患儿(其中急性期16例,恢复期10例)及20例健康对照儿童的血清sVCAM-1、IL-6、IgE水平。结果:①紫癜性肾炎组较单纯过敏性紫癜组、正常对照组血清sVCAM-1均明显升高,且急性期高于恢复期及正常对照组,差异均有显著性意义(P均<0.001),肾炎组、单纯组分别与对照组比较,血清IL-6、IgE水平升高,差异有显著性意义(P<0.01),恢复期较正常对照组血清sVCAM-1、IL-6水平无很大变化,差异无显著性意义(P均>0.05)。②紫癜性肾炎组、单纯过敏性紫癜组sVCAM-1水平随血清IL-6、IgE水平升高而增加(相关系数分别为0.35、0.38,P均<0.01)。结论:sVCAM-1参与了过敏性紫癜、紫癜性肾炎的发病过程,且可反映其病情程度。

Abstract:Objective:To explore the pathogenic role of soluble vascular cell adhesion molecule-1 (sVCAM-1) in Henoch-Schonlein purpura nephritis(HSPN) and the relationship among sVCAM-1,11^6 and IgE in children with HSPN. Methods:Three different groups were studied:27 cases with HSP,35 cases with HSPN(16 of them in acute stage and 10 in recovery) and 20 children as normal controls. Serum levels of sVCAM-l,IL-6 and IgE were determined by EUS A. Results -.The levels of serum sVCAM-1 in HSPN cases were significantly increased compared with HSP cases and normal controls respectively( P < 0.01) .And the serum levels of sVCAM-1 in acute stage HSPN children were significantly higher than those in recovery stage HSPN ones( P < 0.001) .The levels of serum IL-6 and IgE were significantly higher in HSPN and HSP cases than those in normal controls, respectively (P < 0.001). There were positive correlation of serum sVCAM-1 with IL-6 and IgE between HSPN,HSP groups (r were 0.35^0.38,respectively P <0.01) .Conclusion:These results suggest that sVCAM-1 participates in the pathogenesis of HSPN and the level of the serum is correlated with the seriousness of HSPN and HSP.
