全胃切除治疗贲门癌106例分析

本文讨论106例贲门癌施行全胃切除术。其中55例手工吻合作对照组,51例采用改进吻合器吻合为研究组,改进手术以空肠代胃重建消化道,以吻合器行端侧吻合,代胃呈“P”型长臂法,保证血运,并发症明显减少。证明运用吻合器吻合是一个较易掌握的技术。本组并发症发生率5.7％(6例),无手术死亡,全组5年生存率36.7％。

Treatment of the Gastric Cardiac Cancer by Total Gastrectomy

A retrospective study of 106 patients with adenocarcinoma of the gastric cardia treated by total gas- trectomy,including 55 by routine method as control and 51 by improvised method as study group is reported. The inno- vated total gastrectomy was to take the jejunum instead of a gastric pouch for reconstruction of alimentary tract.End-to- side anastomosis was carried out by stapling device,forming a reversed "P" gastric substitute and ensuring blood supply as well as less postoperative complications. This technique is simple and easy to perform. The frequency of complication was 5. 7% without operative mortality in this series. The 5-year survival rate was 36.7%.
