姜黄素改善肾移植受者胰岛素敏感性研究

摘    要:目的研究姜黄素对肾移植患者胰岛素敏感性的影响。方法研究对象为2010年9月至2012年10月南方医科大学珠江医院收治的60例肾移植患者。所有患者均签署知情同意书,符合医学伦理学规定。按随机数字表随机分为姜黄素治疗组(治疗组)和无姜黄素对照组(对照组),各30例。最后完成研究治疗组26例,对照组30例。术前和术后每周1次评价与比较两组患者的空腹血糖水平和胰岛素敏感性;比较两组入组时诊断糖尿病的患者的疗效;观察姜黄素的不良反应及比较两组患者的血脂变化差异。结果与对照组比较,治疗组在胰岛素抵抗指数、空腹血糖水平方面均呈好转趋势,两组之间有统计学意义(均为P〈0.05)。两组的甘油三酯、胆固醇水平,改善移植后糖尿病疗效等方面比较无明显差别。结论在肾移植受者中应用姜黄素具有改善胰岛素抵抗、降低空腹血糖的作用,在临床中有一定的效果,为移植后糖尿病提供新的防治方法。

Abstract:Objective To study the influence of curcumin on insulin sensitivity in renal transplant patients. Methods Sixty renal transplant patients were selected as objects, who were performed transplantation and followed up in Zhujiang Hospital of Nanfang Medical University from September 2010 to October 2012. Local ethical committee approval had been received and the informed consent of all participating subjects was obtained. According to the random number table, 60 patients were randomly devided into curcumin treatment group ( treatment group, n = 30) and no curcumin control group ( control group, n = 30). Finally, 26 cases of treatment group and 30 cases of control group completed the trial. The fasting glucose levels and insulin sensitivity were evaluated and compared between 2 groups before operation and once a week after operation. The curative effect on diabetes was compared. The adverse reaction of curcumin was observed. And the change of blood fat was compared between two groups. Results Compared with control group, insulin resistance index and fasting glucose levels improved in treatment group with significant difference between two groups ( all in P 〈 0. 05 ). There was no significant difference in triglycerides level, cholesterol levels and curative effect on posttransplant diabetes. Conclusions Curcumin can improve insulin resistance and decrease fasting glucose in renal transplant recipients. It is a new treatment for post transplantation diabetes mellitus.
