支气管动脉栓塞治疗肺结核咯血的研究

目的研究支气管动脉栓塞治疗结核咯血的作用与预后。方法因结核咯血住院的患者65例,其中男性40例,女性25例,平均年龄49岁。62例行支气管动脉造影术以及BAE,3例无法实施BAE。对所有患者随访1年。结果 62例患者即刻止血,5例患者一个月内再出血,8例患者一个月后再出血。并发症包括2例支气管动脉穿孔;1例栓塞剂回流;2例肩痛;1例胸痛。结论 BAE具有止血快、疗效好等优点,是治疗咯血的一种有效疗法,它能让不适合外科手术的结核患者避免外科手术治疗。

Analysis of bronchial artery embolization in the treatment of hemoptysis of pulmonary tuberculosis

Objective To study the curative effect and prognosis of bronchial artery embolization （BAE） in the treatment of pa- tients with hemoptysis of tuberculosis. Methods 65 patients with hemoptysis were selected in this study, including 40 men and 25 women with a mean age of 49 years old. All patients were treated with bronch-arteriography, and 62 of them were successfully treated with BAE. All of them were followed-up for 1 year. Results The cessation of hemoptysis was immediately obtained in all patients received BAE, and there were 5 patients reoccurred bleeding within 1 month and 8 patients 1 month after the treatment. The complications of BAE included bronchial arterial perforation （2 eases） , reflux of embolie material （1 case） , omalgia （2 cases） , and thoracalgia （1 case）. Conclusion BAE has the advantages of instant haemostasis and satisfactory clinical result, which is a useful therapy to control both acute and chronic hemoptysis. BAE may help to avoid surgery in patients who are not qualified for surgical.
