不同方案治疗90例急性呼吸窘迫综合征的对比研究

目的探讨血必净辅助治疗急性呼吸窘迫综合征的临床价值。方法本院诊治的90例急性呼吸窘迫综合征患者随机分成2组:对照组45例,采用机械通气+盐酸氨溴索方案治疗;实验组45例,采用机械通气+盐酸氨溴索+血必净方案治疗。观察比较两组的心率、血气分析、APACHEⅡ评分、Murray评分、Marshall评分以及炎症因子指标水平的变化情况。结果实验组治疗后的HR、PaO2、PaO2/FiO2指标水平显著性优于对照组(P<0.05);实验组治疗后的APACHEⅡ评分、Murray评分以及Marshall评分显著性低于对照组(P<0.05);实验组治疗后的IL-6、IL-10指标水平及IL-6/IL-10均显著性小于对照组(P<0.05)。结论临床应用血必净辅助治疗急性呼吸窘迫综合征能明显改善血气、降低炎症因子水平、减轻肺损伤,提高器官功能。

Comparative study on different methods in the treatment of patients with acute respiratory distress syndrome

Objective To investigate the clinical value of Xuebijing injection as the adjuvant treatment of patients with acute re- spiratory distress syndrome. Methods 90 patients with acute respiratory distress syndrome were randomly divided into two groups, the control group （ n = 45 ） and the treatment group （ n = 45 ）. The control group was given mechanical ventilation and ambrexol hydrochlo- ride, and the treatment group was additionally given Xuebijing injection on the basis of the control group. Heart rate, blood gas analysis, APACHE II score, Murray and Marshall score, and inflammation cytokines levels were observed and compared between the two groups. Results The levels of HR, PaO2 and PaO2/FiO2 were better improved in the treatment group than in the control group （ P 〈 0. 05 ）. The scores of APACHE I], Murray and Marshall were significantly lower in the treatment group than in the control group （ P 〈 0. 05 ）. The lev- els of IL-6, IL-10 and IL-6/IL-10 were significantly lower in the treatment group than in the control group （ P 〈 0. 05 ）. Conclusion The clinical application of Xuebijing injection can significantly improve blood gas analysis, reduce inflammation cytokines and lung injury, and improve their organ functions in the treatment of patients with acute respiratory distress syndrome.
