手术内固定对钝性胸外伤导致连枷胸合并呼吸困难的观察

目的探讨手术内固定对钝性胸外伤导致连枷胸合并呼吸困难的临床疗效。方法选取我院收治的32例胸外伤致连枷胸合并呼吸困难患者,分为观察组17例和对照组15例,对照组采用骨折肋骨处进行包扎、牵引等常规治疗方法,观察组行手术内固定治疗,对比两组患者术后ARDS、肺部感染、住院时间、治愈率以及6个月后复查肺功能指标。结果观察组治愈16例,治愈率94.1%;对照组治愈13例,治愈率86.7%,其中有1例死于重症呼吸衰竭。观察组术后并发症明显低于对照组;6个月后复查结果发现,观察组肺功能各项指标均高于对照组(P<0.05)。结论手术内固定可以减少胸外伤致连枷胸合并呼吸衰竭患者的并发症,具有满意的短期和长期疗效。

Clinical observation of internal fixation in the treatment of patients with flail chest complicated with dyspnea caused by blunt chest trauma

Objective To explore the clinical effect of internal fixation in the treatment of patients with flail chest complicated with dyspnea caused by blunt chest trauma. Methods 32 patients with flail chest complicated with dyspnea caused by blunt chest trauma were randomly divided into the observation group （17 cases） and the control group （15 cases）. The control group was given the conven- tional treatment, such as bandage and traction around fracture of rib, and the observation group was treated with internal fixation. ARDS, lung infection, duration of hospital stay, cure rate and lung function indexes 6 months after the treatment were compared between the two groups. Results The cure rate was 94. 1% （ 16 cases） in the observation group and 86. 7% （ 13 eases） in the control group, of which 1 patient died of severe respiratory failure. The incidence of complications was significantly lower in the observation group than in the con- trol group. The improvement of lung function was more pronounced in the observation group than in the control group （ P 〈 0. 05 ）. Con- clusion Internal fixation can reduce the complications in the treatment of patients with flail chest complicated with dyspnea caused by blunt chest trauma, with a satisfying short-term and long-term curative effect.
