经尿道前列腺电切术治疗高龄高危前列腺增生

目的探讨高龄高危前列腺增生(BPH)经尿道前列腺电切(TURP)的安全性及围术期的处理。方法本研究选择30例患者,年龄70～93岁,平均75岁。病史均为进行性排尿困难,平均7.2(2～18)年;术前IPSS 22～34分,平均(26.6±1.8)分;生活质量评分(QOL)5～6分,平均(5.2±0.4)分;最大尿流率0～8 ml/s,平均(6.5±2.2)ml/s。本组患者前列腺质量50～120 g,平均75 g。结果 30例TURP患者手术时间45～115 min,平均60 min;切除腺体质量32～78 g,平均45 g。术中出血50～400 ml,平均100 ml。有4例患者术后输血200～400 ml,术中顺利,无电切综合征等并发症发生。结论经尿道前列腺电切是治疗高龄高危前列腺增生有效、安全性高且并发症少的方法。

Transurethral resection of prostate for elderly patients with benign prostatic hyperplasia

Methods Thirty patients aged 75 years （70 -93 years）. Patients all bad pro- gressive dysuria, with an average history of 7.2 （2 -18） years; IPSS were 22 -34 points preoperative, average （26. 6± 1.8 ） points; The quality of life （QOL） scores were 5 -6 points, average （5.2 ± O. 4） points; The maximum flow rate was 0 - 8 ml/s, average （6. 5 ± 2. 2）ml/s. The prostate weight was 50 - 120 g, average 75 g. Results The surgery time was 45 - 115 min in 30 patients with TURP, average 60 rain; The removal of the gland weighted 32 -78 g, an average of 45 g. The blood loss was 50 -400 ml, an average of 100 ml, 4 patients had blood transfusion 200 -400 ml. The surgery were all successful, and there were no complications such as TURS. Conclusion Transurethral resection of prostate is an effective, safe method and has fewer complications for elderly patients with benign prostatic hyperplasia.
