机械通气对老年急性左心衰的治疗作用

目的探讨老年急性左心衰患者应用机械通气的疗效。方法59例老年急性左心衰患者,在常规抗心衰治疗症状无效时进行机械通气治疗。观察临床重要生理参数改变、病情好转率等。结果与治疗前比较,治疗后6小时临床症状有明显改善,在各项生命体征及血气分析指标中,心率(HR),呼吸频率(R)、平均动脉血压(MBP)、动脉血氧分压(PaO2),动脉血二氧化碳分压(PaCO2)和碱超(BE)均有改善(P<0.05),动脉血氧饱和度(SpO2)和尿量有显著改善(P<0.01)。59例患者经机械通气辅助呼吸后,心力衰竭症状得到不同程度控制,抢救成功率71.19%。结论机械通气能及时有效地缓解老年急性左心衰患者的低氧血症、高碳酸血症和呼吸功能障碍,明显提高老年急性左心衰患者的治疗抢救成功率。

Mechanical Ventilation Therapy(MV)for Aged Critical Illness Patients with Acute Left Ventricular Failure.

Objective To explore the efficacy of mechanical ventilation （MV） in treatment of aged critical illness patients with acute heart failure （HF）. Methods 59 aged patients with acute heart failure were treated with MV at correct time when conventional Anti- HF treatment could not relieve obviously. Major physiological data and improvement rate in clinical were observed and statistic analysis was made to see the efficacy of MV in treatment of aged critical illness patients with acute heart failure. Results After 6h with MV treatment, clinical symptoms （life sign） improved obviously. Clinical symptoms and analysis of blood gas showed that HR, R, MBP, PaO2, PaCO2 and BE improved significantly （ P 〈 0.05）. SpO2 and capacity of urine improved significantly even more （ P 〈 0.01 ）. The acute heart failure was controlled. Survivable rate was 71.19%. Conclusions For the aged critical illness patients with acute heart failure, MV could relieve anoxemia, hypereapnia and respiratory iusufficiency effectively, and obviously increase success rate of emergency care.
