双器械吻合联合全直肠系膜切除对低位直肠癌保肛手术的临床分析(附48例报告)

目的：探讨在全直肠系膜切除的基础上，利用双器械吻合技术行低位直肠癌保肛手术的可行性。方法：对48例低位直肠癌行低位或超低位吻合加直肠系膜全切除的治疗情况进行总结分析。结果：48例中，低位吻合7例，超低位吻合41例，占85．4％；全组无手术死亡，无出血、吻合口漏及吻合口狭窄等严重并发症。切端（远切端）癌残留1例，2年内复发转移2例，全组病例6个月内大便次数均在5次／d以下，3次以下者37例，占77．8％，无大便失禁。结论：在全直肠系膜切除的基础上，利用双器械吻合技术行低位直肠癌保肛手术是安全的、可行的。

THE FEASIBILITY OF ANUSPRESERVATION OPERATION ON LOW POSITION RECTAL CANCER USING DOUBLE STAPLING TECHNIQUE COMBINED WITH TOTAL MESORECTAL

Objective:To probe in the value of double stapling technique(DST) combined with total mesorectal excision (TME) in treatment of sphincte rsaving operation on low position rectal cancer Methods:48 cases of low position rectal cancer operated with low or ultralow colonrectum anastomosis combined with TME were analyzed,including the radical cure effect,the operative mergencesymptom and the function of excreting feces Results:41 cases were performed on with ultralow colonrectum anastomosis (85 4%),7 cases were performed on with low colonrectum anastomosis There was no operation death, leakage of anastomotic opening,narrowness of anastomotic opening and incontnence of feces in the group There was tumor remaining in 1 case and another case recurred in 2 years Conclusion:Using DST combined with TME for therapy of low position rectal cancer is safe and feasible
