关节镜下修补肩袖损伤的疗效与相关性因素分析

目的：探讨关节镜下修补肩袖损伤的治疗效果及相关性影响因素。方法：2006—01—2009—05行关节镜下修补肩袖损伤的患者30例，其中男16例，女14例，年龄39～72岁，平均56岁，预后因素包括年龄、病程、关节活动范围、肌力、撕裂大小、肌腱回缩程度和质量。结果：根据UCLA评分，优23例（76．7％），良5例（16．7％），差2例（6．7％）。术后患者肌力明显改善（P〈0．05）；患者的年龄、撕裂大小及回缩的程度等因素明显影响预后（P〈0．05）；术前的病程、症状和关节活动范围对治疗效果没有明显影响（P〉0．05）。结论：关节镜下修补肩袖损伤创伤比较小，是治疗肩袖损伤的有效方法，但需要正确操作及不断提高关节镜操作技术，并在选择患者时仔细考虑影响疗效的因素．以提高治疗效果。

Effect and Dependence Factors Analysis of Arthroscopic Rotator Cuff Repair

Objective To evaluate the results of arthroscopic rotator cuff repair and determine prognostic factors. Methods From January 2006 to April 2009, 30 shoulders of 16 men and 14 women ranging from 39 to 72 years were evaluated, whose mean age was 56 years old. Prognostic factors included age, symptom duration, range of motion, muscle strength, tear size, retraction and quality of tendon. Results According to the UCLA scale, the results were satisfactory in 23 shoulders （70.7%）, moderate in 5 （12.2%）, and poor in 2 （17.1%）. Postoperative improvements in muscle strength were significant （P〈 0.05）. Age, tear size, retraction and quality of tendon, and fatty degenerative changes were found to significantly affect un- satisfactory results （P〈0.05）. Preoperative symptom duration, range of motion, and muscle strength were not effective on the results （P〉0.05）. Conclusion Arthroscopic rotator cuff repair is a successful treatment for rotator cuff tear. The results may be more satisfactory through improving arthroseopic skills and taking prognostic factors into consideration in patient selection.
