手三阳经穴针刺与经皮穴位电刺激在上肢功能重建中的临床对比研究

目的:在客观准确评价脑卒中患者上肢运动功能的基础上,观察手三阳经穴针刺与经皮穴位电刺激对上肢功能重建的影响。方法:选择符合纳入标准的脑卒中患者36例,随机分为2组,分别给予手三阳经穴针刺(对照组)与经皮穴位电刺激(治疗组)。以上肢表面肌电信号(sEMG)特征值、Fugl-Meyer功能评定表及Barthel ADL指数评价表评价各组治疗前后的患侧上肢运动功能。结果:2组患者治疗前后的sEMG的峰值(MAX)、积分肌电值(IEMG)、Fugl-Meyer及Barthel ADL评分值均有提高,治疗组与对照组疗效相比有明显提高。结论:手三阳经穴针刺可改善中风后偏瘫患者的肢体功能,经皮穴位电刺激亦为一种有效的临床康复治疗方法,可促进脑卒中后上肢功能重建。且临床中具有无创、无痛、操作方便等优点。

Clinical comparative study of acupuncture on hand Sanyang meridians and transcutaneous electrical point stimulation on the reconstruction of upper limb function

Objective:To observe the effect of acupuncture hand Sanyang meridians and transcutaneous electrical point stimulation on the reconstruction of upper limb function.Methods:36 cases of stroke patients,randomly divided into two groups,were given acupuncture on hand Sanyang meridians(control group)and transcutaneous electrical point stimulation(treatment group).Evaluate the affected upper limb motor function before and after treatment by the eigenvalue of upper surface EMG(sEMG),Fugl-Meyer function evaluateforms and Barthel ADL index.Results:sEMG peak(MAX),integrated electromyogram(IEMG),Fugl-Meyer and Barthel ADL all increased before and after treatment in the two groups.Treatment group had a apparent increase compared to control group.Conclusion:Acupuncture on hand Sanyang meridians can improve the limb function of patients with hemiplegia after stroke and transcutaneous electrical point stimulation is also an effective clinical rehabilitation treatment.It can enhance the reconstruction of upper function after stroke.It has advantages of non-invasive,painless,easy to operate in clinic.
