窒息新生儿凝血功能状态的研究

目的探讨窒息足月、早产新生儿凝血功能的变化。方法检测10例正常儿、44例窒息足月儿、32例窒息早产儿凝血指标。结果与对照组比较,窒息足月儿PT、APTT、FIB、PLT无显著差异,D-D明显升高,且与窒息程度正相关。窒息早产儿与窒息足月儿比较PT、APTT延长,FIB、PLT降低,D-D无明显差异。结论窒息足月儿大都存有高凝为特征的前DIC或DIC早期;窒息早产儿更易发生DIC,且有向低凝期发展的趋势。

Study on the state of coagualation in newborn with asphyxia

Objective To expolore the changes of coagualation state in asphyxiated newborns and its clinical significance. Methods The levels of prothrombin time (PT), activated partial thromboplastic time (APTT), Fibrinogen (FIB),D-dimer(D-D),platelet account(PLT)in serum are measured in 10 normal full-time newborns,44 asphyxiated term infants, 32 asphyxiated preterm infants.WTHZ]Results In the group of full-time neonates with asphyxia,the plasma levels of D-D increased significantly, and positively correlated with chokey severity,but PT、APTT、FIB、PLT varied slightly;Compared with asphyxiated term infants, PT、APTT prolonged,PLT lowerd,the plasma levels of FIB declined distinctly,while those of D-D increased slightly in the group of asphyxiated premature infants.Conclusion The findings of this study suggest that Pre-DIC characterized by hypercoagulation is present in asphyxiated newborn infants. DIC befallened easierly in asphyxiated premature newborns,and inclined to hypocoagulation stage.
