腹腔镜监视下腹壁小切口子宫肌瘤剥除术的临床应用

摘    要:目的探讨腹腔镜辅助下腹壁小切口剥除子宫肌瘤的安全性和可行性。方法对69例子宫肌瘤拟行手术的病人分为两组:研究组32例,在腹腔镜监测下行腹壁小切口子宫肌瘤剥除术;对照组37例,行剖腹子宫肌瘤剥除术;观察两组的手术时间、术中出血量、术后肠功能恢复等情况。结果腹腔镜监测下腹壁小切口子宫肌瘤剥除术手术时间平均为90.6±25.4min,明显高于剖腹手术时间;术中的出血量两组无显著性差异;研究组术后病人胃肠道功能恢复时间及住院时间明显短于对照组;两组均无术中、术后并发症。结论腹腔镜监测下腹壁小切口子宫肌瘤剥除术创伤小、术后病人胃肠道功能恢复时间及住院时间短、切口美观,可作为治疗子宫肌瘤的微创技术之一。

Hysteromyomectomy with Small Cut on Abdominal Wall under the Monitoring of Laparoscope

Abstract:Objective To discuss the safety and feasibility of hysteromyomectomy with small cut on abdominal wall under the monitoring of laparoscope. Methods 69 cases with hysteromyoma were divided into 2 groups: treatment group ( n = 32), receiving hysteromyomectomy with small cut on abdominal wall under the monitoring of laparoscope and control group ( n = 37 ), receiving laparohysteromyomectomy. The operation time, bleeding in operation and the recovery of intestinal function after operation in both groups were observed. Results The average operation time of hysteromyomectomy with small cut on abdominal wall under the monitoring of laparoscope was 90.6±25.4 minutes, much shorter than that of laparohysteromyomectomy; there was no significant difference between the 2 groups in bleeding in operation; the time for intestinal function recovery and hospitalization of treatment group was much shorter than that of control group; no complications in and after operation were found in both groups. Conclusion Hysteromyomectomy with small cut on abdominal wall under the monitoring of laparoscope is of such advantages as small trauma, short time of intestinal function recovery and hospitalization.
