19876例腹腔镜胆囊切除术麻醉并发症分析

目的探讨腹腔镜胆囊切除术（LC）麻醉并发症原因及预防处理措施.方法回顾性分析1991年4月至2009年3月19876例LC临床麻醉资料.结果共发生麻醉相关的并发症3543例（17.83%）,其中牙齿脱落28例,呼吸心跳停止5例（0.025%）,急性肺水肿6例（0.03%）,心率失常185例（0.93%）,术中低氧血症165例（0.83%）,误吸3例（0.015%）,显著苏醒延迟183例（0.92%）,急性心肌梗死1例（0.005%）,脑血栓或脑溢血32例（0.16%）,恶心呕吐623例（3.13%）,声音嘶哑362例（1.82%）.结论严格遵守麻醉操作原则,接台全麻送麻醉苏醒室（PACU）,能早期发现和预防并发症发生从而有效地保证患者安全.

Complications of General Anesthesia in Laparoscopic Cholecystectomy (LC): Analysis of 19 876 Cases

Objective To explore the causes,prevention and management of complications during and post general anesthesia in LC.Method The clinical data of 1 9876 patients who accepted general anesthesia undwent LCs from May 1991 to March 2009 at our department were retrospectively analyzed.Results The complication rate was 17.83%（3 543 patients）including tooth loose and extractron in 28 （0.14% ）,cardiopulmonary asystole in 5patients（0.025%）,acute pulmonary edema in 6 patients （0.03%）,cardiac rhythm disorder 185 patients （0.93%）,hypoxemia during anesthesia in 165 patients （0.83%）,acute myocardial infarction in 1 patients（0.005%）,cerebral embolism and hemorrhage in 32 patients （0.16%）,respiratory arrest postextubation in 582 patients （2.93%）,emergence agitation in 1368 patients （6.88%）,postoperative nausea and vomiting in 623 patients （3.13%）,sore hoarseness 362 patients （1.82%）.Conclusions The complications during and post general anesthesia in laparoscopic cholecystectomy can be found early and prevented by strict abiding by the anesthesia operation principles.
