Ghrelin对小儿急性阑尾炎的临床意义探讨

目的：探讨Ghrelin在小儿急性阑尾炎血清和组织中的表达情况。方法选择阑尾炎患儿34例，分别于术前（t1）、术后3天（t2）、术后6 d（t3）采集静脉血；选择肠套叠且无明显腹腔感染患儿为对照组（C组），入院时采集静脉血；测定其白细胞计数、CRP、降钙素原和Ghrelin浓度。按照术后病理结果将急性阑尾炎分为单纯性阑尾炎（A1）、急性化脓性阑尾炎并穿孔（A2）和急性坏疽性阑尾炎（A3）。同时收集阑尾炎、肠套叠患儿手术切除阑尾组织，用免疫组化法测定其Ghrelin含量。结果在急性阑尾炎组，A1、A2、A3血清Ghrelin都低于C，A2、A3标本IOD值低于C。t1、t2 Ghrelin浓度明显低于t3，而t1、t2之间无明显差异。结论检测小儿外周血中Ghrelin浓度，对于判断急性阑尾炎的严重程度价值较小，对病程、治疗、预后的评价有一定应用价值。

The relation between acute appendicitis in children and Ghrelin expression

Objetive To explore the relation between acute appendicitis in children and Ghrelin expression in serum or its appendix tissue. Methods The blood samples of 34 children with appendicitis were collected before the operation,three days and six days after the operation respectively;the blood samples of children with intussusception without obvious evidence of abdominal infection was collected as the Control group;white blood cells,CRP,procalcitonin and Ghrelin concentrations were detected in all these samples.Acute appendicitis was classified into three groups.While the level of Ghrelin in specimens of appendicitis and intussusception were detected by immunohistochemistry. Results In the group with acute appendicitis,A1,A2 and A3 Serum Ghrelin were lower than the control group.The IOD values of A2 and A3 specimens was lower than the control group.t1and t2 Ghrelin concentration were significantly lower than t3.There was no significant differences between t1and t2. Conclusion Ghrelin is of less value in prognosis in the severity of appendicitis,but it Is of a certain value for the course of disease,treatment and prognosis.
