经皮冠状动脉内支架术后体位的研究

目的为减少冠状动脉 (冠脉 )内支架术后患者因卧床时间长并制动而引起的各种不适 ,探讨术后体位与出血的关系。方法对照组 40例冠脉内支架术后患者的体位采用传统方法 ,即冠脉内支架术后患者术侧肢体保持伸直 ,4～ 6h拔除鞘管后予 1kg沙袋压迫穿刺点 6h ,48h后可下床活动 ,实验组 36例在对照组基础上术侧肢体可水平移动 ,若术后 4h内出现排尿困难者可把床头摇起 30°～ 40° ,根据患者需要可予术侧体位和半卧位。结果实验组穿刺点出血与对照组相比无明显差异 (P >0 0 5 ) ;而实验组术后各种并发症明显少于对照组 (P <0 0 1)。结论提高了病人舒适度 ,减少了病人的心理压力和并发症 ,为今后冠脉内支架术后患者体位方式提供了可靠的临床依据

Study on the body position after coronary artery inner bracket operation through skin

Objective To reduce the patient's discomfort due to long period stay in bed and lacking moving after coronary artery inner bracket operation.and to study the relationship between body position after operation and bleeding.Method 40 cases of patients with coronary artery inner bracket operation in control group have routine body position, such as straighten post-operation, puncture point pressure with one kilogram sand bag 4 to 6 hours after pull out the sheath ,then get out of bed to move after 48 hours.36 cases in experiment group, conduct routine body position, plus horizontal moving of limb in operation side ,if dysuresia occurred ,raise bed head 30 to 40 degree and adjust body position to lateral position or semi-reclining position if necessary. Result There is no significantly difference for bleeding in operation puncture point between experiment and control group ( P >0 05) the occurrence of complication after operation in experiment group is significantly low compared with control.Conclusion the new method is comfortable for patients ,and reduce complication and psychological pressure, It provided the useful base for patients with coronary artery inner bracket operation.
