摘    要:目的探讨应用经鼻(面)罩双水平正压(BiPAP)呼吸机治疗慢性阻塞性肺疾病(COPD)合并急性呼吸衰竭(ARF)的临床价值。方法对30例COPD并发ARF的患者在常规治疗的基础上应用BiPAP呼吸机进行无创通气治疗,观察治疗前、治疗2h、治疗7d的临床症状、心率、血压、呼吸频率、动脉血气分析等指标的变化。结果治疗前后临床症状明显改善,pH、氧分压(PO2)、二氧化碳分压(PCO2)、脉搏氧饱和度(SpO2)、呼吸频率、心率的改善差异均有统计学意义(P〈001),血压的改善差异亦有统计学意义(P〈0.05)。结论应用BiPAP呼吸机治疗COPD合并ARF是安全和有效的。

Abstract:Objective To discuss the clinical significance of the application with BiPAP noninvasive ventilation in chronic obstructive pulmonary disease(COPD) patients combined with acute respiratory failure.Methods The 30 COPD patients combined with respiratory failure were treated with traditional methods and plus BiPAP noninvasive ventilation.The changes of the clinical manifestation,heart rate(HR),respiratory rate(RR),blood pressure,blood gas analysis were observed before treatment,after 2 hours and 7 days treatment.Results The clinical manifestation were notably improved and the values of PO2,SpO2,PCO2,HR,RR were significantly changed well(P<0.01),the blood pressure also changed significantly(P<0.05).Conclusion Using BiPAP noninvasive ventilation in COPD patients combined with acute respiratory failure is effective and safe.
