胃癌中内分泌细胞与癌分化程度的关系

本文利用11例胃类癌和13例低或未分化癌的切除标本,探讨内分泌细胞出现的数量与胃癌类型和分化程度的关系。切片均作了嗜银、亲银组化染色,并用四种神经内分泌抗血清(胃泌素、生长抑素、胰多肽、血清素)PAP 法免疫组化染色,4例并作了电镜检查。结果胃类癌每例至少1项阳性,其中4例电镜证实有神经分泌颗粒,低或未分化癌组多数病例各项检查阴性,但4例有不同阳性结果,其中2例阳性细胞数超过50%,可称内分泌细胞癌。表明肿瘤愈原始,愈多向分化。并对胃癌中内分泌细胞与癌分化程度关系,以及胃类癌和胃内分泌细胞癌的特征、名称和组织发生进行了讨论。

Correlation between the endoerine cell and differentiation status of gastric adenocarcinoma

11 specimens of resected gastric carcinoids and 13 rcsect- ed gastric adenocarcinomas with poor differentiation or undifferentiation were analysed.The number of endocrine cell showed in the specimen and its relationship between the histologic type and the differentiation was studied.Paraffin sections were stained by argyrophil reaction and argenta- ffin reaction respectively.PAP method was used to detect the gastrin, somatostatin,serotonin and pancreatic polypeptide.In addition,4 cases were examined by electron microscopy (EM).In cases Of carcinoid one positivity was demonstrated and by EM neuroendocrine granules appeared in 4 cases.Poor differentated or undifferentiated carcinomas all showed negative results except in 4 cases with varied positive demonstration,among which 2 carcinomas contained a high percentage of endocrine cells (>50%) being called endocrine cell carcinoma.It was suggested that the poorer the differentiation,the more the multidirectional differentiation occur.
