过敏性支气管哮喘外周血中白细胞介素17阳性调节性T细胞的表达水平变化及临床意义

摘    要:目的:探讨调节性T细胞(Treg细胞)表达白细胞介素17(IL-17)的特征及临床意义。方法:纳入对屋尘螨过敏的哮喘患者27例作为研究对象,健康志愿者25名作为对照组。采用流式细胞术检测外周血IL-17+Treg/Treg的百分比,及其与第1秒用力呼气容积占预计值百分比(FEV1%)。结果:间歇-轻度哮喘组(n=16)、中重度哮喘组(n=11)、正常对照组(n=25)IL-17+Treg/Treg百分比分别为(7.51±2.90)%、(19.13±9.80)%、(45.30±2.23)%,间歇-轻度哮喘组与正常对照组无统计学差异(P=0.054),中重度哮喘组与正常对照组及间歇-轻度哮喘组之间有明显统计学差异(分别P<0.001、P<0.01)。结论:过敏性哮喘患者外周血中Treg细胞可分泌IL-17,IL-17+Treg细胞可能为Treg细胞向Th17细胞转化的中间态,IL-17+Treg细胞可能在炎症反应过程中起到重要作用,从而影响了哮喘患者的病情。

Abstract:Objective To study the characteristics and significance of expression of interleukin 17 (IL-17) by regulatory T cells (Treg). Methods Twenty seven patients with house dust mites allergic asthma and 25 healthy volunteers served as control were enrolled. IL-17Treg/Treg ratio in peripheral blood was determined by flow cytometry, and forced expiratory volume in one second (FEVI) as the percentage of predicted value (FEVI% pred) was measured. Results The IL-17CFreg/Treg ratio of intermittent-mild asthma (n=16), moderate-severe asthma (n=l 1) and control group (n=25) were (7.51±2.90)%, (19.13±9.8)% and (5.30±2.23)%, respectively. IL-17CFreg/Treg ratio in patients with intermittent-mild asthma and moderate-severe asthma was higher than that in healthy controls (7.51±2.90)% vs (5.30±2.33)%, P=-0.054; (19.13±9.80)% vs (5.30±2.33)%, P〈0.001], and there was also a statistically significant difference between intermittent-mild asthma and moderate-severe asthma (7.51±2.90)% vs (19.13±9.80)%, P〈0.01]. The IL-17Treg/Treg ratio was negatively correlated with FEV pred (r=-0.465, P〈0.01). Conclusions IL-17reg/Treg ratio in peripheral blood was elevated in pa- tients with allergic asthma, and might play an important role in the course of inflammatory response, thereby influencing the severity of asthma.
