石狮市2007-2012年老年人住院疾病谱分析

目的探讨石狮市男性与女性老年人住院疾病谱的特点。方法借助新农合运行数据,以《国际疾病分类》(ICD-10)标准分类,动态分析石狮市2007-2012年住院老年人性别疾病谱。结果该市六年间数据显示:(1)老年人男女比例为1.24:1,60~和70~岁年龄别占90%左右;(2)老年男性与老年女性的疾病谱变动均不大,病种相对集中:前五位疾病构成比均在73%以上;但两者疾病谱不尽相同:循环系统疾病在老年男性和老年女性中均居首位;肿瘤是老年男性第二顺位疾病,老年女性中为第三或第四顺位;呼吸系统和消化系统疾病在老年男性中分别为第三、四顺位,在老年女性中则是在第二至第四顺位间波动;泌尿生殖系统疾病在男性中为第五顺位,而女性则是眼及附器疾病与内分泌、营养及代谢病波动在第五或第六顺位;(3)单病种分析:脑血管病、高血压和冠心病均是最主要的循环系统疾病,老年男性恶性肿瘤比例较高,老年女性患糖尿病和白内障比例较高。结论 2007-2012年,循环系统疾病是石狮市老年人住院疾病谱第一顺位疾病,不同性别老年人疾病谱特点不同,应针对性地开展老年人的疾病预防控制和治疗措施。

Analysis on the diseae spectrums of senior inpatients in Shishi City in 2007-2012

Objective To study the characteristics of disease spectrums of senior male and female inpatients in Shishi City in 2007-2012. Methods Based on the New Rural Cooperative Medical System（ NCMS）,the data about disease spectrums of senior male and female inpatients in Shishi City during 2007 and 2012 were collected and dynamically analyzed. The diseases were classified according to the International Classification of Diseases 10（ ICD-10）. Results According to the data during the six years：（ 1） the ratio of males to females was 1. 24： 1. The old aged 60 years and above occupied around 90%.（ 2） The disease spectrums of the males and females didn＇t change a lot and focused on some particular diseases. The top five diseases occupied more than 73% of all the diseases. The ratios of circulatory system diseases ranked the first in the disease spectrums of both genders. To be more exactly,it was higher in females than in males. The ratio of tumor patients was higher in males than in females. The diseases of respiratory system and digestive system in males ranked the third and the fourth respectively,but they were fluctuated between the second and the fourth places in females. The diseases of urogenital system in males came to the fifth,but eye and adnexa diseases as well as endocrine,nutritional and metabolic diseases ranked the fifth or the sixth in females.（ 3） As for the analysis of each single disease,cerebrovascular disease,hypertension and coronary heart disease（ CHD） were the top three diseases in the circulatory system. The ratio of malignant tumor patients was higher in males than in females,while the ratios of diabetes and cataract were higher in females than in males. Conclusions Circulatory system diseases rank the first in the disease spectrums of the senior inpatients in Shishi City in 2007-2012. The characteristics of the disease spectrums of the senior male and female inpatients are different. Therefore,prevention,control and treatment of the senile diseases should be targetedly c
