肺炎链球菌感染患儿细胞免疫功能分析

摘    要:目的了解肺炎链球菌(SP)感染的肺炎患儿细胞免疫功能变化情况,以便临床合理使用免疫调节剂。方法采用流式细胞仪检测方法,对2012年1月-2013年2月本院小儿科收治的158例SP感染患儿的外周血进行T细胞亚群、B淋巴细胞以及NK细胞进行测定。另选同期来院做健康体检的儿童40例为对照组。结果 158名SP感染患儿外周血中CD3+、CD4+T淋巴细胞明显降低、CD8+T淋巴细胞明显增高、CD4+/CD8+比值明显下降,CD19+B淋巴细胞较对照组高,NK细胞则比对照组低,差异均有统计学意义(P0.01)。结论 SP感染的肺炎患儿体内细胞免疫功能降低,临床上治疗SP感染患儿时,在抗感染的同时适当应用免疫调节剂,可有效防止并发症的发生,提高疗效,缩短病程。

Abstract:Objective To investigate the changes of cellular immunity in children suffered from Streptococcus pneumoniae( S.pneumoniae) pneumonia so as to provide reasonable suggestions for clinical targeted use of immune modulators Methods Flow cytometry instrument was used to detect peripheral blood T-cell subsets,B lymphocytes and NK cells in 158 S. pneumoniae infection children hospitalized in Pediatric Department,the 163 th Hospital of PLA during January 2012 through February 2013. And 40 children who were performed physical examination in this hospital were simultaneously selected as the controls. Results Compared with the control group,the serum levels of CD3+and CD4+T lymphocytes as well as the ratio of CD4+/CD8+in S. pneumoniae-infected children decreased significantly( P〈0. 01),while the serum level of CD8+T lymphocytes increased significantly( P〈0.01). The serum level of CD19+B lymphocytes was higher in S. pneumoniae-infected children than in the control group,while the serum level of NK cells was lower in S. pneumoniae-infected children group than in the control group,with statistically significant differences( P〈0. 01). Conclusions The humoral immune responses of S. pneumoniae-infected children are characterized by the increased serum level of B lymphocytes. It can be a helpful basis for the clinical targeted use of immune modulators.
