磁共振扩散张量成像对早期帕金森病的诊断价值

摘    要:目的:研究帕金森病(PD)患者双侧丘脑和双侧基底节区磁共振扩散张量成像(DTI)的变化,评价DTI技术用于早期PD的诊断价值。方法 :以PD患者26例为PD组,按Hoehn-Yahr分级量表将患者分为PD组1和PD组2;健康志愿者26例为对照组;常规行MR头部平扫和DTI检查,比较双侧丘脑、基底节区的部分各向异常分数(FA)值和表观弥散系数(ADC)值。结果:PD组尾状核和丘脑的部分各向异常分数FA值明显低于对照组(P0.05),PD组兴趣区的ADC值在各组之间、病变双侧之间和对照组之间无明显差异(P0.05),但随着病情级别的增加,ADC值呈增高趋势。结论:磁共振DTI的尾状核、丘脑的FA值有助于PD的早期诊断。

Abstract:Objective: To study the change of bilateral thalamus and bilateral basal ganglia region diffusion tensor imaging (DTI) of patients with Parkinson's disease (PD) to evaluate the early diagnosis value of DTI technology for PD. Methods: 26 patients with PD as group PD, and divided into PD group 1 and PD group 2 according to the Hoehn-Yahr grading scales, while 26 healthy volunteers as control group. All objects were conventional MR scan head and DTI checks. The fractional anisotropy (FA)and apparent diffusion coefficient (ADC)values of bilateral thalamus and basal ganglia region were compared between the two groups. Results: FA values of the caudate nucleus and thalamus in PD group was significantly lower than that in the control group (P〈 0.05). ADC values of interest areas in PD group in both PD group 1 and 2 , the both pathological sides and the control group were no significant difference (P〉0.05), but with the increase of illness level, an increased trend of ADC values. Conclusion:It is help for the early diagnosis of PD on caudate nucleus and thalamus FA values of magnetic resonance DTI.
