脑利钠肽在老年心功能衰竭诊断中的作用

目的 探讨脑利钠肽在老年心功能衰竭诊断中的作用.方法 选取冠状动脉粥样硬化性心脏病所致左心室收缩功能衰竭患者98例,依据美国纽约心脏病学会NYHA标准予以分级,放射免疫法测定血浆（brain natriureticpeptide,BNP）水平,心脏彩色多普勒超声诊断仪测量左室射血分数（left ventricular function,LVEF）;Swan-Ganz球囊漂浮导管检测肺动脉压;通过急性生理学及慢性健康状况评分系统（APACHE-Ⅱ）予以系统评分.结果 左心室功能衰竭组BNP水平明显高于对照组,具有显著统计学意义（P＜0.05）.血浆BNP水平与LVEF呈负相关（P＜0.05）,与肺动脉压力呈正相关（P＜0.05）.结论 血浆BNP水平可作为老年心力衰竭的诊断指标之一.

The value of levels of brain natriuretic peptide in diagnosis of elderly chronic heart failure

Objective To explore the role of brain natriuretic peptide （BNP） in diagnosis of elderly chronic heart failure. Methods 98 patients with left ventricular systolic function failure caused by coronary atherosclerotic heart disease were invastigated. Cases were graded according to the New York Heart Association NYHA criteria. Plasma levels of BNP were measured by radio immune assay. Doppler echocardi ography was performed to measure left ventricular ejection fraction （LVEF）. Pulmonary artery pressure （PAP） was detected by Swan-Ganz catheter. The system scoring was finished by using acute physiology and chronic health evaluation （APACHE-Ⅱ ）. Results The levele of plasma BNP in left ventricular failure group was significantly higher than that in the control group （P〈0.05）. The plasma levels of BNP had a negative correlation with LVEF （P〈0.05）, and the levels of BNP had a positive correlation with PAP （P〈0.05）. Conclusion The levels of BNP may be one of the diagnostic indicators of elderly chronic heart failure.
