2型糖尿病合并肾病患者相关危险因素分析

目的：分析2型糖尿病肾病(DN)相关危险因素。方法：随机抽取2型糖尿病(NIDDM)患者194例，根据24h尿白蛋白排泄率(UAER)将其分为3组并进行相关分析。结果：相关分析显示，UAER与年龄、病程、餐后血糖(PBG)、收缩压(SBP)、舒张压(DBP)、血清尿酸(SUA)、甘油三酯(TG)、纤维蛋白原(FIB)，餐后胰岛素(PINS)、胰岛素抵抗指数(IR)和24h尿尿酸(UUA)等多变量呈正相关。回归分析表明，病程、24hUUA、TC、FIB、IR为DN的独立危险因素。患者尿白蛋白排泄量与病程、血压、尿NAG等相关。结论：DN的危险因素有多种，主要与胰岛素抵抗、血胆固醇(TC)、FIB及24hUUA排量增加有关。NIDDM患者尿白蛋白排泄量随病程延长、血压升高、尿NAG增加而明显增加，且与糖尿病视网膜病变关系密切。

Analysis of relational risk factors of diabetic nephropathy in type 2 diabetic patients

Objective: To evaluate the risk factors of diabetic nephropathy in type 2 diabetic patients. Methods:194 type 2 diabetes mellitus patients were divided into three groups according to urine albumin excretion rate(UAER),and correlation analysis were carried out.Results: There were positive correlations between UAER and age, illness duration,PBG,SBP,DBP,SUA,TG,FIB,PINS,IR,24h uric acid of urine(UUA). Illness duration,24h UUA,TC,FIB,IR were independent risk factors of diabetic nephropathy. There were positive correlations between excretion of albuminuria in NIDDM and course, blood pressure etc, too. Conclusion: There are many risk factors in diabetic nephropathy. It is possibly associated with IR, high TC, high FIB, and increased 24h UUA. Excretion of albuminu -ria increase following by course elongation, raised blood pressure and NAG in urine, and it is closely related with DM-retinopathy.
