连续性肾脏替代治疗在肾移植术后重症肺炎治疗中的应用

摘    要:目的探讨连续性肾脏替代治疗(CRRT)对肾移植术后重症肺炎的治疗效果. 方法对19例患者进行综合治疗的同时,应用CRRT,10~12h/d,共3~5 d. 结果成功救治14例,死亡5例;疗程结束后17例体温降为低热,心率减慢,大部分呼吸困难减轻;外周血白细胞(WBC)和中性(N)百分比显著下降(P<0.01), 淋巴细胞(L)和单核细胞(M)百分比显著上升(P<0.01);血气各项指标SaO2、PaO2、pH、BE、HCO-3均有显著改善(P<0.01). 结论 CRRT可以调节机体的免疫状态,改善症状,对肾移植术后重症肺炎的治疗效果肯定,是一种有效的辅助治疗措施.

Abstract:OBJECTIVE To evaluate the app li cation of continuous renal replacement treatment(CRRT) of severe pneumonia afte r kidney transplantation. METHODS Nineteen patients with severe pne umonia after kidney transplantation were included in this research,which underw ent CRRT in addition to combined treatment. RESULTS Fourteen patients were rescued su ccessful.After 3-5 days of treatment,17 patients become low fever,the heart rate slowed down,and dyspnea in most patients improved.Blood examination also improved apparently,including that white blood cell and neutrophil decreased o bviously(P<0.01),and SaO_2,PaO_2,pH,BE,HCO~-_3improve d simultaneously(P<0.01). CONCLUSIONS CRRT can use in the treatme nt of severe pneumonia after kidney transplantation to adjust the body immune co ndition.It is an effective adjuvant method in rescuing severe pneumonia,which can bring out satisfactory outcome.
