十二指肠镜在胆总管结石治疗中的应用

目的探讨十二指肠镜在胆总管结石治疗中的应用疗效及安全性。方法自2007年4月～2009年12月以来我院B超或ERCP诊断为胆总管结石的50例患者,结石直径〈1.2 cm,结石为1～2枚。用针刀行乳头预切开,在X线透视下用取石网或气囊取石。结果全组50例患者47例行乳头预切开。选择性胆管插管成功47例,取石成功45例,另外3例为巨大憩室内乳头且结石〉1.0 cm,因而放弃。术后并发轻型胰腺炎2例,无出血及胆道感染,开腹手术5例。结论十二指肠镜在胆总管结石治疗中的应用是安全有效。

Duodenal endoscopy in the treatment of colninon bile duet stones

Objective To investigate the duodenal surgery in the treatment of common bile duct stones efficacy and safety.Methods From April 2007～Dec 2009 in our hospital diagnosed as B-ultrasound or ERCP in 50 patients with common bile duct stones,stone diameter less than 1.2cm,stone for 1～2 pieces.Nipple line with the needle-knife pre-cut,in the X-ray taken under the stone with the net or airbag to take stones.Results All 50 patients with 47 routine pre-cut the nipple.The success of selective bile duct cannulation in 47 cases,45 cases successfully stone taken the other three cases of a giant open room nipple and stones 1.0cm,and thus to give up.Light postoperative pancreatitis 2 cases,no bleeding and biliary tract infection,laparotomy in 5 cases.Conclusion Duodenal endoscopy in the treatment of common bile duct stones is safe and effective.
