三维适形放射治疗和化疗综合治疗局限期小细胞肺癌

摘    要:目的 评价三维适形放射治疗配合化疗综合治疗局限期小细胞肺癌的意义和疗效。方法 将65例局限期小细胞肺癌患者随机分为2组,观察组32例,行EP方案化疗,3周为1周期,共行6个周期,放疗在化疗1个周期后进行,先行常规放射治疗,照射野包括原发病灶和纵隔,照射40Cy后对原发灶行三维适形放射治疗,原发灶总剂量累积达70Gy。放疗期间,化疗照常进行;对照组33例,仅行EP方案化疗,共6周期。结果 观察组有效率明显优于对照组(93.75% Vs72.7%,P〈0.05),1、2、3年生存率观察组和对照组分别为68%、46%、25%和40%、22%、10%。结论 三维适形放射治疗和化疗综合治疗局限期小细胞肺癌是一种安全有效的手段.但长期生存和远期并发症尚需进一步随访。

Abstract:Objective To assess the efficacy and feasibility of chemo-radiotherapy concomitant therapy and 3-dimensional conformal radiotherapy (3DCRT) with limited stage small cell lung cancer (LSCLC).Methods Sixty-five patients with LSCLC were randomly divided into two groups:the treatment group(32 patients)and control group (33 patients);the treatment group received 6 cycles EP(cisplatin,Etoposide)chemotherapy and 3 weeks was a cycle.The conventional radiotherapy was given after the first cycle chemotherapy,then the 3DCRT and the total dose of the primary tumor was 70Gy.The control group only received 6 cycles EP chemotherapy.Results The efficacy of the treatment group was magnificantly higher than the control group (93.75% Vs 72.7%,P<0.05).Conclusions Chemo-radiotherapy concomitant therapy and 3DCRT is an effective and safety way for LSCLC.
