颈动脉体瘤的诊断与治疗临床分析

目的探讨颈动脉体瘤的临床诊断治疗的最佳方案。方法对6例颈动脉体瘤患者分别行术前颈总动脉造影及B超检查，以明确诊断，同时提出颈动脉体瘤术前Matas试验前后的经颅多普勒（TCD）检测方法，以提示颅基底动脉环侧枝循环是否建立，为是否能行手术治疗提供可靠的依据。结果TCD检测，除1例患者已有部分代偿外，余5例患者经Matas压迫实验后，健侧大脑中动脉血流速及峰值明显升高，峰值的增幅大于10％，患侧的大脑中动脉血流速及波峰值基本达到健侧波峰值的80％，提示颅基底动脉侧枝循环已建立。结论颈动脉造影是诊断颈动脉体瘤重要方法，彩色多普勒检测有一定的临床价值。TCD检测是判断大脑中动脉的血流速度及颅基底动脉环是否建立，指导临床能否手术。而手术的术式最好拟定三种方案，视术中所见。

Diagnosis and Treatment for Carotid Body Tumor

Objective To investigate the diagnosis and treatment for carotid body tumor (C.Methods 6 cases of carotid body tumor were diagnosed by receiving carotid arteriography,ultrasonic examination and transcranial color doppler (TCD) before and after Matas test.Results TCD showed that collateral circulation of basilar artery had been established after Matas test,speed and peak value of health lateral middle cerebral artery (MCA) increased,peak value increased 10%,speed and peak value of affected lateral MCA was 80% of those of health lateral.Conclusions Carotid arteriography is an important method of diagnosing carotid body tumor,and TCD could definite the establishment of basilar artery circulation.
