明斯特大学前瞻性心血管积分表对经皮冠状动脉介入治疗术后患者心脏不良事件的预测效果

目的探讨明斯特大学前瞻性心血管(prospective cardiovascular munster,PROCAM)积分表对经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)术后患者发生心脏不良事件的预测效果,并确定其临界值。方法采用方便抽样法选取哈尔滨市某三级甲等医院行PCI术的患者1 094例,于介入治疗前24h收集相关资料,术后对患者心脏不良事件的发生情况进行随访,采用受试者操作特征(receiver operating characteristic,ROC)曲线评价PROCAM积分表对心脏不良事件的预测效果。结果随着PROCAM积分表得分增加,PCI术后患者心脏不良事件的发生率呈增高趋势;ROC曲线分析显示,曲线下面积为0.736(P<0.01),最佳临界值为42分。结论 PROCAM积分表可以用来预测PCI术后患者心脏不良事件的发生风险,以便对处于不同危险等级的患者进行针对性干预,从而更有效地指导患者控制危险因素。

Predictive Value of Prospective Cardiovascular Munster Scoring Scheme for Adverse Cardiac Events after Percutaneous Coronary Intervention

Objective To test the predictive effect of prospective cardiovascular munster（PROCAM） scoring scheme for adverse cardiac events after percutaneous coronary intervention（PCI）,and to determine its critical value.MethodsA total of 1094 participants undergoing PCI treatment in a class-III level A hospital in Harbin were selected to obtain all of the relevant information 24 hours before the PCI treatment.Then the occurrence rates of adverse cardiac events were followed up.The receiver operating characteristic curve（ROC） was used to evaluate the effects of the PROCAM scoring scheme on the prediction of cardiac adverse events.ResultsThe incidence of the adverse cardiac events tended to increase with the increase in the scores of PROCAM scoring scheme.The ROC curve analysis showed that the area under the curve was 0.736（P0.01）,while the critical value was 42.ConclusionThe PROCAM scoring scheme can be used in predicting adverse cardiac events after PCI to perform interventions by different degrees for patients at different risk levels so as to guide patients more effectively to control the risk factors.
