三种方法治疗轻型膝骨关节炎临床疗效的对比

目的探讨三种不同疗法对轻型膝关节骨关节炎(OA)的临床疗效。方法将选定的240例膝关节OA患者随机分为曲安奈德加关节腔灌洗组(A组)、玻璃酸钠组(B组)和关节镜组(C组)。三组患者均以西乐葆(塞来昔布)为基础治疗,口服每次200mg,每天1次,共7d。A组关节腔内注射曲安奈德20mg,每两周1次,两次为一疗程;B组关节腔内注射玻璃酸钠2ml,每周1次,5次为一疗程;C组在关节镜下进行有限清理术后行关节腔内注射玻璃酸钠。以症状、体征为观察指标,评价各组近、中期疗效。结果三种疗法对轻型膝关节OA均有一定的疗效。近期疗效:三组相当;中期疗效:C组最高,A组、B组相当。结论关节镜有限清理术结合关节腔内注射玻璃酸钠这种方法,在这三种疗法中具有起效快、疗效显著且较稳定的特点,值得进一步研究和推广。

The effects of three methods in treating mild knee osteoarthritis

Objective To discuss the effects of three methods in treating mild knee osteoarthritis. Methods The selected 240 patients of knee osteoarthritis were devided into three groups at random： Group A, B and Group C. All the patients took Celebrex capsule as basic treatment for 7 days, 200 mg once per day. Group A： triamcinolone acetonide 20 mg was injected into knee joint cavity once in two weeks, two times for each course. Group B： sodium hyaluronate 2 ml was injected into knee joint cavity once per week, five times for each course. Group C： sodium hyaluronate 2 ml was injected into knee joint cavity after the limited debridement by arthroscopy. The recent and mid-term effects of all groups were evaluated by observing the symptoms and the signs of the patients. Results All the treatments did have some certain effects on the patients. The recent effects of the three groups were equivalent; the rank of mid-term effect of the three groups was CA=B. Conclusions Injection of sodium hyaluronate into knee joint cavity after the limited debridement by arthroscopy is the better one among the three, and it should be given further study and be popularized.
