成都市三级综合医院神经内科门诊就诊者抑郁/焦虑患病率调查

目的通过流行病学调查,研究成都市三级综合性医疗机构神经内科门诊就诊者抑郁/焦虑障碍现况和神经内科医师的识别及处治现状。方法随机抽取三级综合医院神经内科门诊的调查日期,使用焦虑抑郁量表(Hospital Anxiety and Depression Scale,HADS)和患者15项健康状况量表对门诊就诊者进行评估,所有HADS评分≥8分的就诊者由精神科医师用国际神经精神科简式访谈问卷(Mini-international Neuropsychiatric Interview,MINI)进行评估确定诊断。结果 454例神经内科门诊就诊者加入了调查,有253例就诊者HADS≥8分,占55.73%,其中192例就诊者(75.9%)完成了MINI量表的测定,诊断为抑郁/焦虑的患者有119例,神经内科门诊就诊者抑郁/焦虑障碍的患病率为26.2%,发病年龄集中在30～45岁(40.1%),其次为18～30岁(23.4%),主诉最多为头昏、头痛(48.7%),其次为失眠(10.1%)、上下肢麻木(5.2%)、肢体瘫痪无力(2.5%)、尿频(8.8%)、心悸(6.8%)。神经内科医师对患者精神系统疾病的认识率仅有25.7%,患者愿意到精神科接受治疗仅占24.7%。结论三级综合性医疗机构神经内科门诊就诊者抑郁/焦虑患病率高,而神经内科医师对患者精神系统疾病的识别率较低。
