下睑缘切口组织瓣联合自体脂肪转移修复颧部凹陷畸形

目的:探讨下睑缘切口入路组织瓣联合自体脂肪转移修复颧部软组织凹陷畸形的方法。方法:根据凹陷部位可采用两种修复方法:①凹陷部位在颧脂垫以上:采用下睑缘切口入路,松解瘢痕挛缩的眼轮匝肌;褥式缝合两断端,释放眶隔内脂肪;填充在凹陷区皮下眼轮匝肌与眶隔之间,缝合固定;②凹陷部位在颧脂垫区域时:采用下睑缘切口入路,剥离到凹陷区域皮下;充分松解深层的瘢痕粘连组织,拉拢周围脂肪组织、筋膜等形成软组织瓣,褥式缝合。结果:术后随访6月~2年,11例患者对外形和功能满意。颧部软组织凹陷畸形基本消失。结论:下睑缘切口入路组织瓣联合自体脂肪转移是修复颧部小面积软组织凹陷畸形的有效方法之一。

The repairing of zygomatic depression deformity with tissue flap combined with autologous fat transplantation through lower eyelid edge incision

Objective To explore the method of the repairing of zygomatic soft tissue depression deformities with tissue flap combined with autologous fat transplantation through lower eyelid edge incision route.Methods We can take two methods to reparie it according to the parts of depression：①the projection of depression above zygomatic fat pad under the skin.Through lower eyelid edge incision route,release the orbicularis oculi muscle with scar and contracture.Mattress suture both ends,before releasing the orbital fat,fix and fill the orbital fat between orbicularis oculi muscle and orbital.②the projection of depression in zygomatic fat pad under the skin.Through lower eyelid edge incision route,strip till the depression under the skin.Fully release the deep scar adhesion tissue and then Draw the surrounding fat tissue and fascia forming tissue flap,mattress suturing them.Results All 11 cases got a better aesthetic result.The patients were followed up for 6 months to 2 years with satisfactory results with good appearance and function.The depression deformity of zygomatic soft tissue has disappeared.Conclusion It is one of good methods to repair zygomatic small soft tissue depression deformity with tissue flap combined with autologous fat transplantation through lower eyelid edge incision route.
