Mirizzi综合征的腹腔镜治疗及术中处理方法

目的探讨腹腔镜治疗Mirizzi综合征的手术方法及术中处理方法。方法回顾性分析2007年1月-2011年12月16例Mirizzi综合征的临床资料,并对其手术方式及术中处理方法进行总结。结果按照Csendes分型,本组病例分为Ⅰ型10例,Ⅱ型3例,Ⅲ型1例,Ⅳ型2例。本组患者所行手术方案为胆囊切除、胆囊大部切除、损伤胆管修复、T管引流术或Roux-Y胆管空肠吻合术。结论在腹腔镜下采取不同手术方式对于Mirizzi综合征I型、部分II型是可行的、安全的,术中需要准确诊断Mirizzi综合征,并对其准确分型,避免胆管损伤。

Laparoscopic treatment and treatment methods during operation of Mirizzi syndrome

Objective To evaluate the effect and method of laparoscopic cholecystectomy（LC） for Mirizzi syndrome.Methods To retrospectively analyze clinical data of 16 cases with Mirizzi syndrome form January 2007 to December 2011,and make a summary to operating methods and treatments during operation.Results To differentiate the patterns of all patients by Csendes standard,patternⅠ for 10 cases,pattern Ⅱ for 3 cases,pattern Ⅲ for 1 cases,pattern Ⅳ for 2 cases.All 16 patients endured cholecystectomy,mostpart-cholecystectomy,bile track reconstruction,T-tube drainage respectively or Roux-Y major postoperative anastomosis.Conclusion With carefully operating,patternⅠ and pattern Ⅱ of Mirizzi syndrome can be managed smoothly by laparoscopy in differentways,we need an accurate diagnosis Mirizzi syndrome,and accurate the type,avoid injury bile duct.
