阵发性心房颤动射频消融术预后的影响因素分析

目的探讨阵发性心房颤动射频消融术预后的影响因素。方法阵发性心房颤动行环肺静脉消融术患者68例,测定消融术前患者体质量指数(BMI)、P波离散度及左房容积指数(LAVI),根据随访结果分为成功组56例,复发组12例,分析2组相关指标的差异。结果复发组BMI、P波离散度及LAVI高于成功组,环肺静脉消融后,成功组P波离散度及LAVI较消融前明显减小,且明显低于复发组(P<0.05),而复发组2指标差异无明显变化,Logistic多因素逐步回归分析显示P波离散度及LAVI是阵发性心房颤动行环肺静脉消融术复发的危险因素(分别为OR=1.263,95%CI 1.018¹.676;OR=6.372,95%CI 3.2681.676;OR=6.372,95%CI 3.268¹8.348,P均<0.05)。结论 P波离散度及LAVI是阵发性心房颤动行环肺静脉消融术复发的影响因素。

Analyze the prognostic factors of radiofrequency ablation of paroxysmal atrial fibrillation

Objective To investigate the prognostic factors of radiofrequency ablation of paroxysmal atrial fibrillation . Methods Paroxysmal atrial fibrillation circumferential pulmonary vein ablation were performed in 68 patients, measured pa-tients＆#39;body mass index （BMI）, P wave dispersion and left atrial volume index （LAVI） before ablation, according to follow-up results, patients were divided into successful group with 56 patients, recurrence group with 12 cases, analyze the differences of related indicators between two groups .Results BMI, P wave dispersion and LAVI in recurrence group were higher than in the success group, after circumferential pulmonary vein ablation , compared with before ablation, the P-wave dispersion and LAVI were significantly reduced in success group , and significantly less than the recurrence group , and these 2 indexes were revealed no significant changes in the recurrence group , Logistic multivariate regression analysis showed that P -wave dispersion and paroxysmal atrial fibrillation is a risk factor LAVI circumferential pulmonary vein ablation recurrence （OR =1.263, 95% CI 1.018 -1.676; OR =6.372, 95% CI 3.268 -18.348, P ＆lt;0.05, respectively）.Conclusion P wave dispersion and LAVI are the recurrence factors in patients with paroxysmal atrial fibrillation who received circumferential pulmonary vein ablation.
