疏肝培土生金法治疗慢性阻塞性肺疾病合并焦虑抑郁临床观察

目的:研究疏肝培土生金法对慢性阻塞性肺疾病(COPD)合并抑郁焦虑患者生活质量、肺功能的影响。方法:采用汉密尔顿焦虑量表(HAMA)及汉密尔顿抑郁量表(HAMD)(17项)测量心理状况,将存在轻中度焦虑抑郁症状的69例COPD患者随机分为疏肝培土生金法组(24例)、舍曲林组(23例)及呼吸操组(22例),治疗4周。治疗前后检测患者的各项肺功能指标,并用圣乔治呼吸问卷(SGRQ)评价生活质量。结果:3组治疗前后HAMA及HAMD评分降低,肺功能均有改善,生活质量评分均降低,差异有统计学意义(P<0.05),在HAMA及HAMD评分方面,舍曲林组较其他两组差异有统计学意义(P<0.05),在疾病影响方面,中药组较其他两组差异有统计学意义(P<0.05)。结论:疏肝培土生金法能改善COPD患者的焦虑及抑郁症状,提高生活质量。

Clinical Observation of Stagnancy in Liver-Spleen-Invigorating and Lung-Nourishing Decoction Treating Depression and Anxiety in Patients with Chronic Obstructive Pulmonary Disease

Objective ： To investigate the influence on quality of life and efficacy of Stagnancy in liver - spleen - invigorating and lung - nourishing method treating the depression and anxiety in patients with chronic obstructive pulmonary disease （COPD）. Methods：A total of 69 patients with mild or moderate depression or anxiety COPD were randomly divided into traditional Chinese medicine group （ n = 24） and sequlin group （ n = 23 ） and respiratory function training group （ n = 22） for 4 weeks. The effi- cacy and side effects were assessed using the Hamilton anxiety scale （HAMA）, Hamilton depressive scale （HAMD）, the St. Geor- ges＇ respiratory questionnaire（SGRQ）. Results：The scores of HAMA and HAMD and SGRQ were decreased significantly （P 〈 0. 05 ）. There was a significant change in the scores of HAMA and HAMD in sequlin group. There was a significant improvement in the scores of social activities （ P 〈 0.05 ） in traditional Chinese medicine group. Conclusion ： Stagnancy in liver - spleen - invig- orating and lunz - nourishing method effective in the treatment of depression and anxiety in the patients with COPD.
