保脾手术治疗脾外伤52例分析

报告1979～1991年收治的52例保脾手术,占同期313例脾外伤手术的16.6%。其中脾修补34例;修补加脾动脉结扎2例;脾部分切除9例;脾修补后未能止血而改行脾切除5例;自体脾组织大网膜移植2例。由于脾脏的重要生理机能,应重视其损伤时的保脾手术。保脾手术并不比切脾风险大、并发症多。保脾术式的选择应根据脾损伤程度。文章还讨论了各种术式的要点。

Splenic Salvage on 52 Cases of Spleen Trauma

Fifty five cases of splenic salvage for spleen trauma from 1979 to 1991 are reported and analysed. They constituted 16.6% of 313 cases who underwent splenic operation in the period of time studied. Splenic repair was performed on 34 patients, repair and splenic artery ligation on 2 partial splenectomy on 9,splenic repair followed by total splenctomy for failure of hemostasis on 5, and splenic antotransplantation of greater omentum after splectomy on 2.The splenic salvage should be emphasized for the important physioligical function of the spleen. The methods of splenic salvage should depend on tranumatic degree of the spleen.
