注射性臀肌挛缩症的机制及治疗

目的分析臀肌挛缩症的发病机制并指导治疗。方法对43例注射性臀肌挛缩症患者,全部行手术治疗,术中对挛缩带进行必要的松解,并随时检查髋关节活动度,达到要求即可,防止过度松解、过度切除,术后早期行功能锻炼。结果所有患者切口均达到甲级愈合,无一例出现切口感染、神经血管损伤等并发症。随访6个月～2年,平均1年,患者步态均恢复正常,膝下蹲试验阴性,手术效果满意。结论注射性臀肌挛缩症由于婴幼儿时期长期反复肌肉注射药物引起,不同部位肌肉受累,其临床表现不完全一致。经手术松解引起症状的臀肌挛缩带,术后早期行功能锻炼,能取得满意的疗效。

Study of mechnism and treatment of the injective gluteal muscle contracture disease

Objective To analyze the mechanism and treatment of the injective gluteal muscle contracture(IGMC).Methods 43 patients of IGMC were treated in our hospital.There were 24 males and 19 female cases and they were 9.2 years old(average,6 to 22 years).The history of disease was 5 years(average,2 and half to 11 years).All cases were operated after relative examinations in hospital.In operation we emphasized the location and size of contracture zone which limited the activity of the hip joint.The contracture limiting the hip activity must be soluted and cut off.But the operator should avoid soluted too much.Functional exercise should be carried out just after operation.Results All cases were healing well and there was no complications such as infection or injury of nerve and vessel.All patients were followed 1 year(average,6 months to 2 years),they all got normal gait and be satisfacted to the operations.Conclusion IGMC were caused by long-term injected intramuscular during infant or children period.The clinical situations correlated to the different muscles involved in.The patients should be operated and got functional exercises.In operations the contracture limiting the hip activity must be soluted and cut off.The results of treatment were satisfaction.
