负压封闭引流联合藻酸盐敷料治疗难愈性烧伤创面的效果

目的探讨负压封闭引流联合藻酸盐敷料治疗难愈性烧伤创面的临床疗效。方法选取100例难愈性烧伤创面患者,按数字表法随机分为观察组(n=50)和对照组(n=50)。对照组给予负压封闭引流联合医用海绵材料;观察组给予负压封闭引流联合藻酸盐敷料,比较两组创面分泌物细菌培养阳性率、换药次数、创面愈合时间、新鲜肉芽生长时间、临床疗效。结果治疗后第1、7、14天,观察组创面分泌物细菌培养阳性率分别为92.0%(46/50)、26.0%(13/50)、2.0%(1/50),对照组分别为90.0%(45/50)、48.0%(24/50)、14.0%(7/50)。第1天时,两组创面分泌物细菌培养阳性率比较无统计学差异(P0.05);第7、14天时,观察组创面分泌物细菌培养阳性率显著低于对照组(P0.05)。观察组换药次数、创面愈合时间及新鲜肉芽生长时间分别为(5.1±1.2)次、(28.2±4.3)d、(15.4±2.7)d,对照组分别为(7.2±1.5)次、(35.1±3.8)d、(22.3±3.5)d,组间比较差异具有统计学意义(P0.05)。观察组和对照组治疗有效率分别为92.0%(46/50)和64.0%(32/50),组间比较差异具有统计学意义(P0.05)。结论负压封闭引流联合藻酸盐敷料治疗难愈性烧伤创面,可明显减少创面细菌感染,促进新鲜肉芽组织生长和创面愈合。

Efficacy of negative pressure drainage combined with alginate dressing in refractory burn wound

Objective To evaluate the efficacy of negative pressure drainage combined with alginate dress-ing in refractory burn wound .Methods One hundred patients with refractory burn wound were divided into the ob-servation group(n=50) and the control group(n=50).The control group was treated with negative pressure drain-age combined with medical sponge material .The observation group was treated with negative pressure drainage com-bined with alginate dressing .The bacteria culture positive rate of wound secreta , dressing changing times , wound healing time,fresh granulation growth time and clinical efficacy were compared between the two groups .Results At 1d,7d and 14d after treatment,the bacteria culture positive rates of wound secreta in the observation group was 92.0%(46/50),26.0%(13/50) and 2.0%(1/50),respectively;while those of the control group was 90.0%(45/50),48.0%(24/50) and 14.0%(7/50),respectively.At 1d after treatment,the bacteria culture positive rates of wound secreta in the two groups were not statistically different (P>0.05).At 7d and 14d,the bacteria cul-ture positive rates of wound secreta in the observation group were significantly less than those of the control group (P<0.05).The dressing changing times,wound sealing time and fresh granulation growth time of the observation group was (5.1 ±1.2)times,(28.2 ±4.3)d and(15.4 ±2.7)d,respectively ;while those of the control group was (7.2 ±1.5)times,(35.1 ±3.8)d and(22.3 ±3.5)d,respectively,and the differences were statistically significant (P<0.05).The clinical efficacy of the observation group and the control group was 92.0%(46/50) and 64.0%(32/50),respectively,and the differences were statistically significant (P<0.05).Conclusion Negative pressure drainage combined with alginate dressing in refractory burn wound can obviously reduce the wound bacterial infection and promote the fresh granulation tissue growth and wound healing .
