人工血管支架置入术治疗创伤性假性动脉瘤和动静脉瘘的 初步体会(附4例报告)

摘    要:目的:介绍人工血管支架置入术(PSG)治疗创伤性假性动脉瘤及动静脉瘘的初步体会。方法:本组4例,其中锁骨下动脉、颈内动脉及肾动脉假性动脉瘤各1例、髂外动静脉瘘1例。均采用经皮人工血管支架置入术治疗。结果:3例假性动脉瘤患者分别置入1枚人工血管支架后假性动脉瘤(PA)被完全隔绝。1例动静脉瘘(AVF)置入2枚支架后漏口被完全封堵。血管造影及临床随访8-50个月,3例支架通畅,1例肾动脉闭塞。结论:PSG是治疗创伤性PA及AVF的简单、安全有效方法。

Abstract:Objective To study primarily treatment of traumatic pseudoaneurysm and arteriovenous fistula with placement stent graft. Methods The research included 4 patients. The lesions included 1 pseudoaneurysm of subclavicular artery, 1 of carotid, 1 of renal artery with stenosis of renal artery and 1 external iliac arteriovenous fistula. All the placement of stent graft was succeeded. Results The 3 patients with pseudoaneurysm each was treated with placement of a imported stent graft successfully. The other arteriovenous fistula was treated with placement of a home made stent graft covered with urethane membrane and an imported stent graft, and the fistula was occluded completely. Four and nine months after placement of the stents in subclavicular and renal artery, angiography demonstrated the patency of the lumen. Five months after placement of stents in renal artery, angiography demonstrated occlusion due to existence of stenosis of the artery at the time of stent placement. The clinical follow up continued for 8 to 50 months. Except for the occlusion of renal artery, no abnormal clinical symptoms occurred. Conclusion Placement of stent graft is simple and effective method for treatment of traumatic pseudoaneurysm and arteriovenous fistula.
